A Client Service Co-ordinator will respond to all participant inquiries regarding coverage, claim status and benefit options. CBA will issue the participant certificates of insurance, pay-direct drug cards,
booklets describing the benefits selected, and will collect the required monthly premiums via automatic bank withdrawals from the participant's personal account on the 1st of each month. Drug claims are handled through the pay-direct card at the pharmacy. Other medical and dental claims are submitted directly to the insurer's local claims office, thus ensuring the confidentiality of health information. Claim payments are sent to the address on the claim form. |
