Option 1: Online Application
You can access our secure online application here. You'll be able to choose the Benefits you want, select any optional coverage your require, and complete the Personal Health Declaration required for approval. Option 2: Manual Application
 Alternatively, you can download the Application Kit (at right or from our Downloads page), which contains a
Program Brochure, the Application Form, and Personal Health Statements. Then complete the required information and attach a VOID cheque to enable Alternative Benefit Solutions Inc. to collect the monthly premiums on the first Thursday of every month.

Mississauga, Ontario L4W 4Y4 Alternative Benefit Solutions Inc. (ABS) will confirm acceptance for coverage as quickly as possible and will issue certificates of insurance, a program booklet and related documents. Coverage will commence on the first of the month following the date of approval.
