Quick and easy enrolment:
Applying for Menuflex MDSA coverage is quick and easy! Option 1: Online Application Employees or individuals can access our secure online application here. You'll be able to choose your MDSA contribution amounts, select any optional coverage
your require, and complete the Personal Health Declaration required for approval. Option 2: Manual Application If you're an employer or self-employed individual, you can download the MDSA
Application Kit, which contains the Application Form, Personal Health Statements (for Optional Benefits) and the Employer Master Agreement. Then follow the appropriate instructions below.
Self-employed Individuals: - Determine the amount of monthly contribution based on the net annual
amount that you wish to have in your MDSA account from the bottom of the 1st page of the MDSA Application Form. Tick the appropriate box. If you are interested in purchasing the Optional Extended Health Care (EHC), or EHC plus Dental please see the top of page two of the Application Form, and indicate if you require Single; Couple; or Family coverage. If you wish to purchase the Optional AD&D, Critical Illness Benefits or Disability Insurance,
please indicate the amount of coverage required on the Application.
 - If purchasing the Optional EHC, EHC plus Dental or Disability Insurance you must also submit a fully completed Personal Health Declaration (PHD), ensuring that you have provided full details of any questions
answered "Yes", for both yourself and any dependents on page 2 of the PHD. The same PHD is used for all Optional coverage except Critical Illness, which has a separate form.
Finally, complete the MDSA Administrative Services
Agreement, and attach a VOID cheque to enable Alternative Benefit Solutions Inc. to collect the MDSA contributions and any Optional Benefit premiums on the first Thursday of every month.

Alternative Benefit Solutions Inc. (ABS) will
confirm acceptance for coverage as quickly as possible and will issue certificates of insurance, a booklet (also available on this site) and MDSA claim forms. Employer Groups: - An employer wishing to participate in the Program and cover two or more
employees completes the MDSA Administrative Services Agreement for the employer group. Individual employees are not required to sign this Agreement. On the Agreement please indicate the amount of monthly contribution per employee based on the net annual amount you wish to provide to each employee. Remember, you can establish separate Classes for different levels of employee and provide a different MDSA contribution to each. Attach a VOID cheque for premium collection by ABS.
- Have each employee complete an Application Form, indicating the amount of employer contribution to the MDSA, and their choice of Optional EHC, or EHC plus Dental, AD&D, Critical Illness and Disability Insurance. If applying for Optional Benefits, each employee must complete the Personal Health Declaration or, in the case of Critical Illness Insurance, the Critical Illness Evidence of Insurability form.
