| Overall maximum of 80% of the eligible
expense as defined in the Medical Care Benefit of this Policy, subject to a maximum of $50,000. A statement of insurability for the newborn is not required. Medical treatment and services including hospital accommodation are covered when confirmed by a physician to be necessary in respect of childbirth. This includes pre- and post-natal treatment of the mother. Newborn nursery and Well Baby Care is in effect until the newborn reaches 15 days of age. Coverage will not apply where the expected
date of confinement is less than ten (10) months from the Insured Person's original effective date. |

 | | Who is eligible for coverage? Expatriate workers and
their spouses under age 70, and accompanying dependent children from 15 days to under age 18 (24 for full-time students), are eligible for coverage while travelling outside their home country of permanent residence on temporary or permanent foreign assignment, including vacation travel incidental to the assignment, not exceeding 90 days if following the completion of the assignment. (Coverage in home country restricted to 90 consecutive days per trip during periods of Foreign Assignment for
Emergency Medical Care, assuming premium has been paid to include that geographical area.) Foreign nationals outside their home country of permanent residence may also be covered under this Program when on foreign assignment.
In order to be eligible for coverage, the applicant (not necessarily the dependents) must be employed on a full-time basis. Students (unless applicant's dependent) are not eligible. |

travel outside the home country of permanent residence for a minimum of 3 months. Rates in effect at the commencement of a foreign assignment shall apply for the duration of the assignment, subject to a maximum of 12 months. Each expatriate employee must compete a Health Statement for themselves and any accompanying family members to be approved by the insurer before coverage can commence.

| Medical Evidence Requirements |
| Medical evidence of insurability in the form of a Health Questionnaire must be provided by the Employee, and
each eligible dependent accompanying the employee. Application must be made and health evidence supplied prior to departure. Coverage will only commence after approval by the insurer.
Coverage may be subject to specific exclusions or a surcharge based on the health information provided. |
